Job responsibilities


You'll lead the Rehabilitation and Wellbeing
Team in delivering high quality palliative rehabilitation and wellbeing support
to patients and carers. This isn't just about care its about helping people
live fully, with dignity, for as long as they can. You'll bring a holistic
approach to support people to live as well as they can for as long as they can.

You'll work alongside colleagues from a range
of disciplines within the hospice on the inpatient unit, outpatient and
community services and advise the hospices hospital-based team. Sharing your
expertise to help us all deliver the best care we can.

with partners across the health and care system because great end-of-life
care doesn't happen in isolation.










Key Responsibilities:

Clinical Practice & Leadership

You'll work with the team to deliver care that is as individual as the
people we support, always holistic and person centred. As the Lead for the
Rehabilitation and Wellbeing Service, you will be a key member of the team,
providing evidenced based clinical expertise and leadership and will:

Lead and champion the service making sure it is seen, heard and valued
